Our Team

Dr. Simone Blake grew up in Southern West Virginia and moved to Columbus, Ohio to pursue her undergraduate studies. She received a Bachelor of Science degree in Biology and her Doctorate in Optometry from The Ohio State University. Upon graduating in 2002, Dr. Blake began working in a private optometric practice in Upper Arlington, Ohio before establishing Perimeter West Eye Care in 2007. She served a two year term as a Trustee in the Central Ohio Optometric Association and is a member of the Ohio Optometric Association and the American Optometric Association. She is an active participant in iSEE and InfantSEE, both public service programs that promote the importance of eye care for infants and school children. Dr. Blake has an interest in family eye care, including pediatric and infant eye care, contact lenses and diagnosis/treatment of eye diseases. In her spare time, Dr. Blake enjoys traveling, fitness and nutrition, reading and spending time with her family. 

Dr. Kelly (Rarick) Cleveland moved to Dublin in 1990 and is a 1995 graduate of Dublin High School. She attended The Ohio State University for both her undergraduate and graduate studies. She is a 2002 Magna Cum Laude graduate with a Doctorate in Optometry. After graduation, Dr. Cleveland moved to Hartford, Connecticut while her husband completed his dental residency. She then returned to Dublin and began working with a local cataract surgeon. Dr. Cleveland is a member of the Ohio Optometric Society and the American Optometric Society and participates in the InfantSEE and RealEyes programs. Her main interests are family eye care, diagnosis and treatment of ocular disease and geriatrics. Dr. Cleveland currently resides in Dublin with her husband, David, and their three children. Her hobbies include hiking, traveling, reading and competitive game nights with her family.

Aylin is often the first friendly point of contact for patients at Perimeter West. After joining our practice in 2020, she has expanded her knowledge of insurance billing and resides as our chief HIPAA compliance officer. She maintains communication with our patients to be sure we are answering any inquiries or meeting patient needs in a timely and accurate manner. Her meticulous organizational skills ensure a simple, streamlined experience every step of the way.

Hannah acts as the ‘right hand’ to Dr. Cleveland and Dr. Blake. Her role includes performing additional testing, capturing imaging, and updating information to be sure your visit with the doctor is focused on your personalized visual needs. Since joining our practice in 2021, Hannah has taken over maintaining our contact lens inventories and honed her skills on our OPTOS retinal scanner. This ensures the doctors have the tools and knowledge they need to make accurate diagnoses and recommend the best plan of treatment for our patients.

Samantha brings a wealth of experience to our dispensary, ensuring our patients the best frame selections and lens designs for their unique visual needs and lifestyle. As our resident frame buyer she personally curates our frame collection to be sure we have styles that are on trend for our patients of every demographic. Samantha joined our practice in 2014, and wears many hats behind the scenes to maintain the high level of care our patients have come to appreciate over the years. She also works with our vendor representatives and continues her education to ensure we are on the forefront of the optical industry.
