Prepare for Your Exam

Prior To Your Appointment:

  • Please contact our office more than 24 hours in advance of your appointment to reschedule, to avoid a $30 cancellation fee

  • Submit the patient health history form as soon as possible after scheduling your appointment for your comprehensive exam

Please send the office images of the front and back of your major medical insurance cards. Some diagnoses or testing fall outside of routine coverage and can be billed to your medical insurance. If this is not possible please bring the physical cards with you to your exam.

If you have diagnoses that affect your ocular health and ability to see, please contact your previous provider for the most recent record summary. These records can be faxed to 614-789-5758 or sent to info@pweyecare.com. Any previous imaging such as retinal photos or OCT scans can be helpful to add to our records as well.

Bring your most current pair(s) of glasses with you to your eye exam. This includes correction for distance, reading, computer/hobby, and prescription sunglasses.

​​​​​​​If you are a current contact lens wearer, please wear them into your comprehensive exam, and also bring the parameters of your contact lens prescription. A physical copy of the prescription, contact lens boxes, or foils from individual blister packs or photos of the aforementioned can be used to provide this information.
